Pfizer says the FDA accepted a supplemental application for Talzenna plus Xtandi in men with HRR gene-altered metastatic castration-sensitive prostate cancer. The combination already has a US indication in a later, castration-resistant stage of disease.

The reporting

What is known.

The company reported that its TALAPRO-3 Phase 3 trial reduced the risk of radiographic progression or death by 52% against Xtandi plus placebo. The FDA assigned a priority-review action date in the final quarter of 2026.

The assessment

Why it is a possibility.

A favorable pivotal result, an established combination in a related setting and an accepted supplemental filing support the provisional 78% estimate for a 2026 expansion.

The remaining question is regulatory: the FDA may require more analysis, choose narrower wording or delay action. Existing approval for the later-stage group would not count as the new indication.

The countercase

What could prevent it.

Reviewers could seek more evidence or narrow the indication before reaching an approval decision.

The test

How it will be assessed.

Resolves YES if the FDA approves talazoparib (Talzenna) with enzalutamide (Xtandi) for men with homologous-recombination-repair gene-altered metastatic castration-sensitive prostate cancer by 31 December 2026. The existing later-stage indication does not count.
